FLORENCE LOCATION HAS A PHYSICIAN MEDICAL DIRECTOR, PRIMARY CARE NURSE PRACTITONERS, BEHAVIORAL HEALTH, PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER (REMOTE), ACUPUNCTURE (WED), SAME DAY APPOINTMENTS AVAILABLE FOR ESTABLISHED PATIENTS, AND TELEMEDICINE.
Telemedicine is available as a MOBILE or In Office appointment.
We are accepting new patients including Oregon Health Plan - Trillium & Pacific Source as well as, Medicare patients. We are currently covered with most private insurers, accepting Motor Vehicle and Workers Compensation Claims as well.
Please arrive 20 minutes prior to your scheduled appointment time. Please bring picture ID and insurance card.
